  • Check that your webcam/microphone are connected and set up properly.
  • Only one software program can access your webcam at a time. Please close any other software that can be using your webcam, like Skype or Messenger. If the issue persists, close all your open browsers (make sure to save your form first!) and try again.
  • After recording, ensure that your file is 130 MB or smaller.
  • Ensure that your video is two minutes or less in length.
  • If you still have trouble, try using a different network connection, computer, or browser.

How do I upload a video using a webcam on my PC? 

Record a video on your webcam and save it on your local drive.
Navigate to the Video Response page in the form and click the ‘Choose File’ tab, then navigate to the video you saved on your local drive and select the file. Make sure the file was saved in one of the following formats: .avi, .wmv, .mov or .mp4.
Wait until your video file name appears next to the ‘Choose File’ tab and then click the Next page button.

How do I upload a video from mobile phone or tablet? 

To upload a video from mobile phone or tablet, click on ‘Choose File’ tab and navigate to Photo/video gallery and select a video from your gallery.

How do I record a video on MacBook? 

The easiest way to record a video on your Mac is with the built-in QuickTime app. Open your Applications folder to find QuickTime. Once it’s open, go to File > New Screen Recording and then click the Record button.

What would happen if I submitted a video format other then .avi, .wmv, .mov and .mp4?  
Unfortunately, other formats cannot be considered, even though it may appear that they have been accepted by the form.

What is a maximum length of video you can upload?   

We can only accept videos that are 2 minutes in length or shorter.  Though our form will accept longer videos, they will not be considered.
